Going to hospital? Want to be back in your own home recovering as soon as possible? Hospital at Home is a program that lets you receive short-term therapy services at home.
What is it?
Hospital at Home is a hospital substitute treatment program. The program gives you access to health services in the comfort of your home rather than staying in or being admitted to a hospital. The program requires a referral from your treating doctor and is an alternative option to receive treatment like IV antibiotics and wound care at home.
Hospital at Home services can be delivered to avoid going to hospital or after a surgery, rather than staying in hospital for short term treatment.
How can it help?
Hospital at Home can help you recover by assisting with:
- complex and simple wound management
- pain management
- palliative care nursing
- medication management
- IV antibiotic therapy
How does it work?
A range of medical conditions can be effectively and safely managed without a person needing to stay in hospital. Your doctor and health care provider will advise you if your condition or treatment can be effectively and safely managed through the Hospital at Home program.
Your doctor needs to refer you to hospital at home services, if you are eligible and deemed appropriate, you do remain under the supervision of your treating doctor whilst you receive medical services and recovery at home.
If you are having upcoming treatment or surgery in a hospital, let your treating doctor know that you are covered for hospital at home services. If your doctor believes at home treatment would be suitable, they can simply refer you to one of our contracted providers who will set you up and develop a care plan that’s right for you.
Your doctor will know where the best place is for you to recover so access to the program is dependent on their referral.
Peoplecare are lucky enough to have a number of healthcare providers to deliver the best care to our members when they need it.
The services are delivered by a dedicated care team including registered and enrolled nurses, allied health professionals such as physios, occupational therapies and medical officers. They will help coordinate care and work with the members GP and other health professionals to provide a safe and effective treatment. You’re in safe hands!
The care plan for treatment is personalised to you and your individual needs for recovery.
If you hold one of our hospital covers and you are covered for the services in a private hospital, you’re fully covered. It doesn’t affect your premium with us either, we just pay the bill.
All your treatment information is kept completely confidential by our health care providers. We won’t have access to any personal health information you give them, but if you’d like to read our Privacy Policy to make sure, click here.
You need to have already served your 12-month waiting period for pre-existing conditions to use this program. This means that if you want to use it in the first 12 months of having or upgrading cover, we’ll need to check with your doctors to make sure it’s not for a condition you already had. If it’s not a pre-existing condition, only the 2-month waiting period applies.
The same restrictions on your hospital cover apply to Hospital at Home services i.e. you won't be eligible to receive any Hospital at Home services for hospital treatments you're not covered for. The provider will let you know if there are any out-of-pocket fees – in most cases there are no costs.
